Shared Care Protocol

We do not accept new or adjusted shared care prescribing in any circumstances

If we are currently prescribing on a shared care basis, please expect this to be reviewed for patient safety reasons.

In the interest of patient safety, many shared care drugs require a number of conditions to be met before transfer can be made to primary care

Shared care is usually declined based on the below points. Please note, this is not an exhaustive list

1. The prescriber does not feel clinically confident in managing the individual patient’s condition and there is a sound clinical basis for refusing to accept shared care

2. The medicine or condition does not fall within the criteria defining suitability for inclusion in a shared care arrangement

3. A minimum duration of supply by the initiating clinician has not been met

4. Initiation and optimisation by the initiating specialist has not been met

5. Shared Care Protocol not received

6. Other (Primary Care Prescriber to provide explanation if there are other reasons why shared care cannot be accepted)

A detailed letter providing reasons for decline will be sent to the requesting clinic
