Before requesting an appointment with one of our clinicians, it may be worth looking at our self-referrals page to check if a surgery appointment is needed for the service you require.

How to Request an Appointment

You can request an appointment with a healthcare professional (GP, Nurse, Pharmacist, Healthcare assistant, Physician associate) the following ways:

  • By contacting us online. It is very simple and quick to fill out. All requests are triaged throughout the day, Monday to Friday.
  • Please note that this service is also available on the NHS App. Once logged on the App, simply click on either “view your messages” or on “messages” on the bottom right corner of the app (Ask your GP surgery a question)

If you are pregnant please ensure you bring your maternity folder / notes with you every time you attend the surgery.

Urgent Appointments

Our urgent appointments are requested through the online triage request form. This form can be completed on the ‘appointments information’ page or on the ‘contact us online’ banner on the homepage.

Your online request will be triaged and you will be signposted to a relevant service such as A&E, NHS111, an on-the-day appointment or an evening/weekend appointment at the extended hours service at Lime Square Medical Centre, North Manchester General Hospital or Victoria Mill.

The service and appointment you are offered will be based on the urgency, medical need and availability once triaged.

Routine Telephone Appointments

Our GPs are allocated time slots throughout the week for these telephone appointments.

Please make us aware if you would like to speak to a specific GP.

Fit Note (Sick Note) Requests

Fit notes are requested as an admin request using the ‘contact us online’ triage form.

You will be asked to self-certify for the first 7 days.

Your first fit note request will require a telephone consultation to discuss and you will be booked in accordingly. If you require an ongoing fit note, you will be booked a non-urgent telephone consultation as the clinician may need to discuss your ongoing request. You may be offered an appointment beyond the date your current fit note ends but you can request for it to be backdated.

Fit notes are usually sent to the patient electronically.

Appointment Reminder Service

The practice will remind you of any appointments via text message.

Did Not Attends

Due to a high number of patients failing to attend appointments, it has become necessary to implement the following policy:

Each time you fail to attend an appointment, we will write to you. If you fail to attend three appointments in a 12-month period, the practice will consider removing you from the patient list.

You will then have to find an alternative GP practice in the local area.

Don’t waste valuable appointments; if you cannot attend your appointment for any reason, please let us know as soon as possible, giving at least 24 hours notice. We can then offer the appointment to someone else.
