Fit notes are requested as an admin request using the ‘contact us online’ form on the home page
You will be asked to self-certify for the first 7 days

Your first fit note request will require a telephone consultation to discuss and you will be booked in accordingly. If you require an ongoing fit note, you will be booked a non-urgent telephone consultation as the clinician may need to discuss your ongoing request. You may be offered an appointment beyond the date your current fit note ends but you can request for it to be backdated
Fit notes are usually sent to the patient electronically
